Wishlist Therapy Dog Program Initial Self-Assessment
The below self-assessment is the first stage of applying for the Sunshine Coast Wishlist Therapy Dog Program.
If you and your dog are interested in exploring this rewarding pathway, the first step is to complete our Prospective Therapy Dog Self-Assessment.
What this form is for
This self-assessment is designed to help us understand:
- A little about you and your background
- Your dog’s health, history, and temperament
- Your goals and the types of environments you’d like to volunteer in
- How much time you’re able to commit
It’s important to know:
- There are no “right or wrong” answers.
- This is an initial screening tool only — it does not guarantee acceptance into a program.
- All dogs will still need to undergo a formal temperament and suitability assessment with a qualified assessor.
